Steps to reproduce:
Plug in usb mouse or memory stick, see errors in syslog about device not
accepting its id.

When i unload all the usb modules i am unable to unload

The memory stick seem to have a problem with the address assigned ?

OPTi cards have always been trouble ... especially the old ones,
which have some nasty chip errata (the ones encouraging that
driver to say "OPTi workarounds unavailable").  Apparently
Apple's Darwin code has workarounds, and they're so nasty that
I expect Linux will never merge them.

Has this card ever worked on any Linux?  If so, when did it
stop working?

Your best bet may be to use a better add-in PCI card.  My
general advice is to avoid ones with VIA chips, but I've
had plenty of success with USB 2.0 cards that have chips
from NEC, ALI, and Philips.
