USB hub fails to enable XHCI devices after wakeup from suspend. Has been happening since at least 3.17.2.

Something that may be related:

Left channel of headphone USB DAC is maxed out after wakeup from suspend. This is happening in both USB2 and USB3 ports. Volume level needs to be adjusted once via alsa or USB device needs to be replugged to reset volume to normal.
