Most recent kernel where this bug did *NOT* occur:
Distribution: Fedora Core 6
Hardware Environment: MacBook Core Duo
Problem Description:
On a resume from suspend-to-mem, machine does not wake up correctly. When I 
wake it up, the screen stays off (both the backlight and the actual display). 
I'm not sure what state it is in as the network doesn't come back. Nothing I 
can do will get it to wake up.
Let me know what logs you need.

Steps to reproduce:
echo "mem" > /sys/power/state
Wake up machine.

what is the latest kernel revision that worked properly,
or has this always failed?

This has always failed.
