- What was the version of the last known working kernel ?
  A dmesg from said working kernel would be welcome.
- Can you reproduce it with 2.6.36.{2, 3} ?

Wild guess of the day : commit 801e147cde02f04b5c2f42764cd43a89fc7400a2
(reverted as of 2.6.36.2).

This NIC worked with kernel 2.6.33.4. I'm attaching the dmesg file.
This NIC doesn't work with kernel 2.6.37.5.
