To understand this properly a quick question from a bystander: the problem you report is new in 5.16.y and 5.15.y work fine?

no. 5.15.2 have some error./

simple to reproduce variantts - 
- fio 4k randrw io=64 jobs=4 aio with direct and sync=0/1. 
- ovirt VM image copy (nfs rdma share for L
 
Intresting! Write queue IOSTAT size is 64/128 total to all LV. If one LV used for expansive copy. I not found where this limit. Now i use multipath between sas and lvm. Its migrate requests and maybe workaround expander restarts. 


Ideeas for issuer:
1. nr_requests = 128 ro in lv?
2. SSD controller trottling or long block TRIM (used UNMAP) waits?
3. SAS3 enclousere -> SAS2 enclousere connection non compatability (sas2 hdd mot used and offline)
4. sas support_ncq = n in driver
5. sas driver phy width and phy count > real phy ports
7. Deadlock/qieie leak in dirver
