The usbmon log shows clearly what the problem is. You have the mini-hub plugged into port 4 of the motherboard's EHCI controller, and you have some other hub plugged into port 3 of the EHCI controller. That other hub doesn't have any devices attached to it, so the kernel suspends it. Then the problem starts. When port 3 is suspended, port 4 stops working also. This must be a hardware bug in your motherboard's EHCI controller. You could try working around it by moving the other hub to a different port or removing it entirely. Alternatively, you could prevent the other hub from being suspended by doing: echo disabled >/sys/bus/usb/devices/1-2/power/wakeup or by turning off CONFIG_USB_SUSPEND in your kernel's configuration. The bug isn't present in any kernel, Ubuntu's or anyone else's. The bug is in the EHCI hardware on Martin's computer. Sorry, please disregard my comment!
