I updated the summary because I think the problem is with the Broadcom card reader device, not the SD card itself. I also see bcm-kernel-feedback-list@broadcom.com, which is not listed for this device, but does look like a fairly generic Broadcom address. This card reader is notorious for being broken. Search for "BCM57765 linux" and you'll see what I mean.
