I have been having the problem of the mouse and keyboard freezing since upgrading my system in May 2012

GIGABYTE GA-Z77X-UD5H LGA 1155 Intel Z77 HDMI SATA
Intel Core i5-3550 Ivy Bridge


Mouse and keyboard stop responding randomly. By using only the usb connections on the motherboard it requires logging in by ssh to restart the computer or hard resetting. Unplugging and plugging the mouse or keyboard in has no effect on the issue.

I added a pci usb controller and this stops working as well but you can unplug and plug the devices back in and continue to use the computer. 

OS: Ubuntu 12.04,12.10 X86-64 -- Ubuntu 12.04.1 i686

Appears that I had faulty hardware causing the problem. Sorry for wasting every-ones time.
