dmesg log of BUG on unplug

Details in the dmesg output; this does not happen on F12's 2.6.32.11-99.fc12.x86_64, but does happen on F13's 2.6.33.2.  Test system is a stock Fedora 13 Thinkpad T42 (32-bit only).

Yay, URB double-frees everywhere!  Fixing...

I've also got another patch that fixes the "don't map memory from the stack BITCH" warning thats in this log too.

Rather than having me bounce all this stuff to the USB list, could I suggest that you prepare a jumbo bug report and email it to linux-usb@vger.kernel.org?

All USB bugs should be sent to the linux-usb@vger.kernel.org mailing 
list, and not entered into bugzilla.  Please bring this issue up there,
if it is still a problem in the latest kernel release.
