If this is still seen on modern kernels then please re-open/update Experienced this on current Ubuntu 14.04 with Kernel 3.13.0-37 (see https://bugs.launchpad.net/ubuntu/+source/linux/+bug/1269412) and now after I switched to CentOS 7.0 with Kernel 3.10.0-123.8.1.el7.x86_64 as well.
